Yes. For applicants who filled out the form DS-11, appointments are scheduled for Mondays, Wednesdays, and Saturdays. Our calendar opens on the 10th of every month and are filled quickly. An appointment can accommodate four family members.

We do not have appointments or walk-ins for December.

Adult applicants (16 and older) must provide the following:

1. Completed, unsigned passport application (DS-11) filled out in black ink

2. Original proof of Citizenship

3. Current proof of Identity

4. Check or Money Order (Application fees)

Minor applicants (15 and under) must provide the following:

1. Completed, unsigned passport application (DS-11) filled out in black ink

2. Original proof of Citizenship (Must show parental relationship)

3. Current proof of Identity (Both parents)

4. Check or Money Order (Application fees)

Both parents must be present for a minor applicant. If this is not possible, please provide a notarized form of consent from the parent who is unable to appear.

There are Application fees that are paid to the US Department of State via check or money order. Note that you will need one for each applicant.

• Minor (15 and younger) - 100

The library issues an Acceptance Facility fee for our services. Note that you can pay via cash, check, or card for these fees.

Yes. We offer Thursdays between 12:30pm-3:30pm as Walk-In passport service for both new applicants and renewal assistance. Walk-In services are not offered during Holiday observances, please check our calendar accordingly.

We do not have walk-ins or appointments in December.

We strongly encourage you to have and submit all required documents as requested by the US Department of State. Submitting partial applications can lead to delays or rejection of your application. Email contact@eastlakelibrary.org if you have any questions.

No. We cannot process renewals, but we can answer renewal questions. If your passport was issued less than 15 years ago, click here to begin the renewal process online.

If you choose to renew via mail, know that you must mail your envelop yourself at a post office. You must submit:

1. Renewal application

2. New photo (we can take one for $12)

3. Most recent passport

4. Check or money order
